A USGS Disk on the Hartstown Road bridge over the Pymatuning Spillway. The disk is worn (like the bridge to which it is attached), and the markings are mostly illegible. The BM does not appear on the
National Geodetic Survey Data Explorer website, but a description of the mark was found in the PennDOT PAMS database:

penndotpams.org/NAD27-NGVD29%20Survey%20Control/Crawford/USGS%20BMs/Linesville.pdf
Described as "Linesville, 2.0 mi S. of, along causeway; at Linesville Spillway "Where the Ducks Walk on the Fish"; 4 ft W. of NW. corner of Highway bridge 356+46; in top of sidewall to Spillway chute; standard tablet stamped 171 MAT 1956 1016"
Height 1016.124 feet
(visit link)

To find the mark, simply make your way to the famous Linesville Spillway, known as the place "Where the Ducks Walk on the Fish" (visit link)
Park in the parking lot for visitors, then walk along the pedestrian pathway until you reach the spillway (about feet). Carefully cross over the Hartstown Road to find the mark on the top sidewall of the spillway chute, just beneath the sign marking the boundary between Pine County and Shenango County.
