This is a Warren pony truss bridge with alternating verticals. There are plaques at each end of the bridge, tilted 45 degrees presumably to fit the girders. They read:

Parking is easier on the north end of the bridge, although the slope on each side may not be something you'll want to tackle. The Lake Creek Cemetery is visible from the bridge, and there's little enough traffic that you can always park there and walk over.
