In 1962, the Maryland Civil War Centennial Commission released and erected a series of official Maryland markers to commemorate specific events revolving about the Civil War. The Battle of South Mountain & other events are mentioned in the marker.

The markers was relocated from a busy National Road intersection to the left of the Central Maryland Heritage League. The marker stands 6 feet from the National Road. The sign is stuck in a brick paved walkway, next to the concrete sidewalk. There is damage to the sign and it looks to have been repaired. The historic marker is on the left or south side of the road if traveling west along the road. The sign reads:
September, 1862, soldiers wounded in the Battle of South Mountain were hospitalized in churches here. July 1863, Gen. Meade established headquarters here as Union Forces pursued Confederates retreating from Gettysburg. July, 1864, Confederate Gen. Early occupied Middletown and collected $1,500 ransom.
