Charles/MGH Station is elevated, being located at the Boston side of the Longfellow Bridge, which carries the Red Line along with vehicle traffic across the Charles River. Trains return to the Red Line tunnel just south of the station.

Charles MGH station is a rather plain elevated affair with two tracks and outside platforms. The platforms are concrete with simple steel framed, copper clad wooden shelters overhead. Platforms were originally about 320 feet long. In the 1980s, the platforms were extended west onto the bridge about 120 feet, the extensions being quite narrow and without shelter.
Future plans for the station include a new, underground terminus for the Blue Line below the elevated station. Currently, there is no direct connection between the Red and Blue lines. As part of a September 2008 lawsuit settlement, the state agreed to fully design the project but is no longer bound by an earlier commitment to build it.
