This is the building used as Karen's Cafe for Moira Kelly's character 'Karen Roe' up until Season 5. The exterior scenes of the building would just show cars driving by or people entering/leaving the Cafe. Most of the scenes actually using the building were interior shots. In Season 5 and later Karen's Cafe turns into Clothes Over Bros run by Sophia Bush's character 'Brooke Davis' after she moves back to Tree Hill from New York and opens up a store for her clothing line.
The front door of this building was covered with a plywood sign advertising the show and tourists could actually walk by the building and look in and see the set (just not during filming). When they do film here they usually have this section of Front Street closed off. I saw fans of the show sitting in chairs on the opposite corner and the star's dressing room trailers are parked a block away on Water Street.
