According to Levi Hayes' Revolutionary War pension file (S4328):
Levi Hayes was a member of Capt. Benjamin Mills' company, Colonel Bazaleel Buzbee's regiment of the Connecticut state troops. He enlisted for nine months "about the last of March or first of April 1780" and served his time out.

He states that he was born in Simsbery, Connecticut in April 1764. When called into service he was living in Simsbery "now called Granby" and lived there until 1805 when he moved to Worthington, Franklin County, and then to Granville, Licking County, Ohio in 1806. [Note that this differs from his tombstone.]

He was awarded a penion of $33 per annum.
