Located in a small park on the edge of the central Wisconsin village of Almond. Could not find a serial number other than the 169 on the tail. Cobra attack helicopter used for destroying enemy armor. I failed to write down the info on the plaque and could not quite read it in the photo. Not 100% sure but it appears to be the actual aircraft and not a replica.
