In his book entitled The Mothman Prophecies, John Keel relates the accounts of his investigation into confirmed sightings of a large, winged creature called Mothman in the vicinity of Point Pleasant, West Virginia, during 1966 and 1967. It combines these accounts with his theories about UFOs and various supernatural phenomena, ultimately connecting them to the collapse of the Silver Bridge across the Ohio River on December 15, 1967.
Additional Requirement?: There is a large statue of the Mothman in the downtown area of Point Pleasant, a Mothman Museum (The world's only), and a festival that takes place every September dedicated to the legend.
 What's the Real Story?: On December 15, 1967, the Silver Bridge collapsed under the weight of rush-hour traffic, resulting in the deaths of 46 people. Two of the victims were never found. Investigation of the wreckage pointed to the cause of the collapse being the failure of a single eyebar in a suspension chain, due to a small defect 0.1 inches (2.5 mm) deep. Analysis showed that the bridge was carrying much heavier loads than it had been designed for and had been poorly maintained.-Silver Bridge
