The milestone simply gives the distance to St. Ann's Square in literally the city centre.

St. Ann's Square is the area around St. Ann's Church which was consecrated in 1712 Manchester was one of the first industrial cities and expanded rapidly, but at the time the church was built it was a small rural village.

When the city expanded the land around the church became St. Ann's Square and the church tower became recognised as the literal centre of Manchester.

It's possible the milestone is also indicating the distance to the centre of Manchester as well as St. Ann's Square.

The milestone has the coat of arms on Manchester at the top.
