The tree and memorial plaque is located at the entrance to the Millburn train station just before the Millburn-Short Hills Volunteer First Aid Squad. Easily missed, look for a singletary tree as you enter or exit the train station parking lot.

Plague is made of bronze and mounted to a boulder.
