After World War II Ludlow, MA saw an influx of Portuguese immigrants. To meet the need of this Portuguese speaking population, in 1948 Bishop Thomas Michael O'Leary created Our Lady of Fatima parish. Construction of the Our Lady of Fatima Church took place in 1949.

A set of steps leads up to a triple arched projecting portico. The center arch is larger than the side arches. The portico has two arches on each side. The main block has three entrances. Directly above the central entrance is a semi-oval ceramic tile mural. On the façade above the portico is a large circular ceramic tile mural depicting the appearance of Our Lady of Fatima.

Rising from the gable roof is a two stage bell tower with a square base and a keystone arch opening. Above is a hip roof covered in roofing tiles and topped by a cross.

The sides of the church have seven four part arched stained glass windows. A stone column separates every second window. The eight bay has a raised projecting entrance covered by a slant roof. At the back are projecting gabled wings. The gable end has a single matching window. Beyond is a projection with a slanted roof and a rounded arch.
