A memorial dedicated in honor of Kathy N. Mazza and three dozen other Port Authority Police Department personnel lost in the attacks on September 11, 2001 is at Ellsworth W. Allen Park in Farmingdale, New York. The monument, on the southwest section of the park, combines several components in a heart shaped garden: a pair of circular elements - brick paved plaza to the east and raised planter bed with a tall flag pole to the west. A black granite monolith, a steel artifact from the World Trade Center, dedicated engraved bricks, and two park benches are incorporated around the patio. The stone stands at the intersection of the two circles and is inscribed on its front face:

My Fallen Flower
Oh the Angels are around me

The poem was written by John Mazza, Captain Mazza’s father. An etched photo realistic portrait of Captain Mazza appears between the first and second lines of the inscription. The names and rank of each PAPD member killed in the line of duty that day are also remembered. Engraved on the stone, divided equally and alphabetically on the left and right edges, those lost are:
