The sign is mounted to a standard, galvanized post next to the sidewalk at the west side of the road beside a two-story, professional office building. The main, circular plaque is in good shape, but all design or text on the accompanying horizontal marker below it is totally faded.

Serving the Plainville community since 1946, this chapter meets monthly at 7:00PM on the first Wednesday at Plainridge Park Casino and Racecourse, 301 Washington Street, Plainville, MA.
