He was born on July 21, 1839 in Berrysburg, PA and died on April 18, 1905 in Sunbury, PA. He attained the rank of Brevet Lieutenant Colonel in the Union Army. His Medal of Honor information and citation is: HOFFMAN, THOMAS W.
