The monument, featuring a stone relief sculpture of a Union soldier at rest, is located in the Quacker cemetery just outside North Collins, NY.

The monument was dedicated in 1909 by the Women's Relief Corps No 22.

Monument is available for viewing during daylight hours. It is
accessible from US 62 south of North Collins, NY.

Text at the base of the monument reads:

To our soldiers dead.
