The Royal is the local hospital for the town of Elsinby. It is run on a shoestring by the administrator, TJ Middleditch. Dr David Cheriton comes for interview at the hospital on a day when the hospital is at full stretch. Mid way through his interview he is despatched to an emergency. A lorry driver has run off the road. Despite his best efforts, Dr Cheriton cannot save the man, particularly as his emergency kit is woefully inadequate. Back at the interview he expresses his concerns about the emergency kit forcefully. He gets the job and the backing of the other staff.
As a sub plot Ken, the dodgy head porter, is trying to install a TV and aerial. His efforts end in near disaster as roof masonry falls through the roof of the operating theatre.
