Standing in front of the Prague Historical Museum in downtown Prague on Jim Thorpe Blvd., this red granite monument documents the amazing athletic accomplishments of Sac and Fox Indian, Jim Thorpe.

At the top of the monument are engravings showing Thorpe in his track uniform and in his football uniform. Also depicted are the NFL Shield, professional baseball logo and the five Olympic rings. The Shield of the State of Oklahoma is located at the center top. The monument is engraved:
