The plaque is of polished marble with an etching ofthe battleship at the top. It reads:

"In honor of the crew of USS Libery - AGTR 5
"In recognition of their courageous and faithful
"service for military action on June 8, 1967
" - the following decorations were awarded -

" 2 Navy Crosses
" 38 Silver - Bronze Stars
" 208 Purple Hearts

On either side of the list of decorations awarded are:

On an angled polished granite base is the following text:
""I commend to every man who sails in the Sixth Fleet the fact that the USS Liberty has become a legend in her own times."
