The blue plaque is in the churchyard of St Magnus the Martyr's church to the north of the tower.

The plaque reads:
Along the top edge:
"The Corporation of"
Along the bottom edge:
"The City of London"
In the centre:
"This churchyard / formed part of the / roadway approach / to / Old London Bridge / 1176 - 1831".

The plaque is in fair condition but is located in a place where it may go unnoticed by many.
