U.S. Geological Survey Benchmark Disk. Couldn't find it in any searches here or on the USGS site. Stamped ET 2 AAS 1959.

Heading north on PA 145 over the Treichler's Bridge, make a right turn at the traffic light at Blue Mountain Drive. Turn right onto Blue Mountain Drive. In 230 feet, turn left onto Long Lane Road. Go approx. 1.2 miles north on Long Lane Road to find the Benchmark Disk on the corner of a driveway.

I emailed USGS, and I encourage anyone who has better luck or more knowledge about these bench marks to help me out in identifying which one it is!
