This station, a U.S. Coast and Geodetic Survey & State Survey standard disk, is located in Milton, NH. It is set in a rock outcrop off the west side of Middleton Road (NH Route 153), about 0.4 mile north from the Farmington/Milton town line.
The NHDOT datasheet description is adequate (accessible at the link below), it got me in the general area okay. The mark had ribbons near it, which was a big help because the color of the disk almost matches the rock, causing it to blend and 'hide'. As found, it's in very good condition.
