This monument is located on the Sammons Family Plot in Johnstown, New York. The monument itself is quite striking with the bust of Simeon Sammons on the front and the coat of arms on the back. On both sides there is a large plaque with the families war history on it. Some of text was very difficult to read so the text has been added below. I apologize if there are any errors in my translation.

The parking here is not good by any means and it does appear to be a family plot. I would advise getting your pic from the side of the road and move on to the next waymark to avoid any problems with traffic. There's a story behind the cannon as well. It killed a man after the war.
