Debido a la excesiva rapidez con la que se construyó y a los hundimientos del terreno debido a numerosos terremotos, sufrió diversas remodelaciones a lo largo de los siglos; Recordamos en particular el que siguió al terremoto de 1688 , obra de los arquitectos Ferdinando Sanfelice , Arcangelo Guglielmelli y sobre todo Carlo Buratti , romano de origen tesino, responsable del aspecto interior actual y de la bóveda de caña. Recientemente ha sido parcialmente restaurado para recuperar su estructura románica original.

De gran valor artístico es la cripta que alberga los restos mortales de San Mateo . La leyenda de la traducción cuenta que las reliquias fueron llevadas a Salerno por Gisulfo I en el siglo X y posteriormente en 1081, cuando se construyó la nueva catedral dedicada al evangelista, fueron colocadas en la cripta destinada a custodiarlas."
