Located on an old bridge abutment A few feet off of the east side PA Rt 53. The bridge crosses over a tributary of Clearfield Creek. Most of the bridge was replaced during the realignment of Route 53. The Disk is located on the east side of the highway on the south end of the bridge Abutment.

Disk markings are: (cast into disk)P.S.H.D. ELEV., (stamped into disk) USGS, 51FPV, 1958.
