This one-story red brick building is located at 627 Mill Street. The main church entrance is on the southern facade. Seven steps lead to an arched entryway consisting of double wooden doors and a peaked arched transom. The writing on the transom reads: "FIRST METHODIST CHURCH Founded by Jesse Walker 1810. Oldest Methodist Church west of the Mississippi." Above the entryway is a datestone that reads: "M.E Church South 1903." A colored glass arched window is above the date stone. Completing the entry tower is a square of venting with a decorative octagon in the middle. To the left of the entryway is a three-part arched stained glass window that dominates the southern facade. Above this large window is a small round window that appears to have been boarded up and painted white.

The church welcome sign is rather simple. It is set in a brickwork planter. At the top is a permanent sign "New Madrid First Uunited Methodist. The central part of the signage can be changed.
