Found on Court Street in Syracuse outside Assumption Cemetary on the North side of the street set in a small concrete circle located approximately halfway between Assumption Cemetary's two main entrances. Disk says "NEW YORK STATE PSM 1642 DEPT OF TRANSPORTATION"
