The disk is a "New York State Power Authority #5" disk, set flush w/the ground, and is located on Middle Road in Henrietta, NY. The disk is found on the S side of the driveway entrance to the Town of Henrietta DOT Highway Equipment Storage facility. The driveway is located on the E side of Middle Road, and the disk is directly adjacent to a wooden marker post. The disk is also approx. 30 ft. N of RG&E pole #39.
