A hand painted electrical box at the intersection of Canal and Smith Streets in Providence, Rhode Island, features the state flag's symbols set on a pastel background.

The featured design is a golden anchor above a blue ribbon surrounded by thirteen golden stars. The state motto "HOPE" is written in the center of the ribbon. The stars wrap around the west, north, south, and top sides of the case. Two light shades of concentric turquoise arches make an abstract ground. The artist/designer Allie Surdovel included her website URL on the north face.

The east side includes the phrase: "PROVIDENCE THE CREATIVE CAPITAL" in black letters across the the upper and lower edges. This labels it as part of the Art Transformer Project, organized by the City of Providence Department of Art, Culture, + Tourism (AC+T).
