A monument of honoring Revolutionary War colonel and originator of the Baldwin apple, Colonel Loammi Baldwin is located on Main Street north of I-95 in Woburn, MA.

A 6' high bronze statue of a colonel is wearing a Continental Army uniform with a tri-corner hat and knee high boots. His right arm is by his side and he is holding a sword, vertically, in his hand. His left arm is bent at the elbow and his fingers are pointing forward. The 6' by 4' square pink granite base contains five bronze plaques. The monument was created by Herbert Adams in 1917 and dedicated April 19, 1920. Bronze plaques on the base provide a biography of Colonel Loammi Baldwin.

The on the front of the base is plaque with a relief profile of Loammi Baldwin and the inscription in raised capital letters:

Below a plaque is inscribed:
