This benchmark is prominently visible on the south corner of the east abutment of the Pawnee Rd. bridge over the Little Papillion Creek through the Glenn Cunningham Park on the north side of Omaha, NE. You'll have to view the benchmark from the road deck as a fence blocks the abutment. That's okay because you can clearly see it. I'm guessing "NDOR" stands for Nebraska Department of Resources." The disk did not have any other designations on it.
