From Smithsonian Art Inventory:

A rectangular stone monument with a bronze relief tablet on one side. The relief depicts Major Sam McColloch and his horse sliding down Wheeling Hill to avoid Indians in the September 1777 siege of Ft. Henry. A pine tree is depicted behind McColloch, with hills visible under the tree branches, and rocks in the foreground.

The bronze tablet is inscribed as follows:
