It appears the plane was built in 1955 as a C-123 Provider, Tail Number 55-4518 (c/n 20179) converted to C-123K. Crashed 25 Jan 1968 on final approach to Duke Field. There was an instructor pilot, a first pilot, a co-pilot, and a flight mechanic on board. They were doing simulated engine-out approaches. All on board were killed.
