From Kellettville, Forest County, go about 0.4 mile south and west on State Highway 666 to a side road left, then go about 5.2 miles southwest on Creek Road to a bridge over Jakes Run and the mark on the left in the top of the southwest wing wall. The mark is a Pennsylvania Department of Highways survey disk stamped ---USGS EGB 1964---. This bridge was formerly the location of survey disk JAKE RUN(MA0954), which was removed when the wing walls of the bridge were replaced.
Condition: Mark found in good condition
