The memorial consists of a relief of Wakefield's profile. Above his profile is the "lamp of maintenance" which is a symbol used by Toc H, an international charity and movement that emerged from a soldiers' club in Poperinge, Belgium, during World War I. Beneath Wakefield's profile is his engraved surname. Further beneath the relief is a stone tablet with the following information:
