The box is located on the Via Luigi Masi side of a residential building whose address is 19 Via Giacomo Venezian, in the Trastevere section of Rome. It's a copy of the decorative shield depicting the head of the Greek mythical character Medusa (who had snakes for hair, and a glimpse of whom could turn humans to stone), painted by Caravaggio and currently on exhibit at the Uffizi Gallery in Florence.
I saw another utility box painted by an artist who used the same cube-shaped signature, and who clearly was doing another copy of a classic painting, on the Via Cavour in the city center, but unfortunately didn't have a camera.
