At the corner of Carlson Drive and H St, between a fire station and a large church. This is near the beginning of River Park residential area. The Yermol family began the plantings.

After their passing, the neighbors added the plaque. There is a fountain, and a number of annuals that keep the weeds down.
There are bearded iris, daffodils, geraniums, and more.
It was a small, neglected piece of ground before the Yermols began to beautify it.
The exact text is shown on the 2nd image.
The fountain was runnng when I went by recently.
