According the the history sign on the building, the International Order of Odd Fellows dedicated the building on April 14, 1908. This is the only surviving example of Romanesque Revival style buildings which once dominated the commercial buildings on early Park Avenue. As originally built, two large arched entrances served the first floor, which housed a grocery store, the Star Theater, and an undertaker. About 1920 the F.W. Woolworth Co. took over the ground floor lease until 1975. Today the Park Avenue Antique Mall occupies the ground floor. The second floor remains active as the I.O.O.F. Lodge Hall with an entrance on the side of the building at 421 A Street. The building was placed on the National Register on Aug. 30, 1984.
