This benchmark is located adjacent to U.S. Hwy. 95, just south of the community of Slate Creek. The cap is located on the south corner of the concrete base of the Wilson Fosskett Memorial. The cap was originally set by the U.S. Coast & Geodetic Survey in 1969 and was marked BM RESET FOSSKETT 1945 NO 4 1969. The original FOSSKETT triangulation station and reference marks were destroyed and this was one of the replacement reference marks when the triangulation station was reset.

DESCRIBED BY NATIONAL GEODETIC SURVEY 1982 1.5 KM (0.9 MI) SOUTH FROM SLATE CREEK. 1.55 KM (0.95 MI) SOUTH ALONG US HIGHWAY 95 FROM THE US FOREST SERVICE RANGER STATION IN SLATE CREEK, IN TOP OF THE SOUTH CORNER OF THE CONCRETE FOUNDATION PAD FOR THE FOSSKETT MONUMENT AT THE SOUTHWEST END OF A PAVED PULLOUT, 0.75 KM (0.4 MI) SOUTHWEST OF MILEPOST 214, 18.9 METERS (62 FT) NORTHWEST OF THE HIGHWAY CENTERLINE, 4.2 METERS (14 FT) SOUTHEAST OF THE TOP EDGE OF THE NORTHEAST BANK OF THE SALMON RIVER, 0.15 METER (0.5 FT) ABOVE THE GROUND. THE MARK IS 0.2 M BELOW THE HIGHWAY.
