To reach the disk, enter Yuma Crossing State Historic Park from parking off of N 4th Ave. Turn North and walk nearly to the river, to the east of the old Water Storage building and west of the Quartermaster's House.
The disk is embedded in a concrete monument a few inches above the surface of the ground. The Stream Gage is north of the disk; it is a small square concrete tower with a railing on top. You can see the door from the disk.
