I believe that the puzzle does not have an appropriate name as it refers to the Church of St. Clement, but the puzzle only shows a semi-circular arch above the entrance of this church.
The dominant feature in the picture is the Circular Chapel of the Assumption of the Virgin Mary, which was built between 1590 and 1597 and served the Italian community in Prague. It was consecrated in 1600. The portico in front of the chapel's portal was modified by F.M. Kanka in 1715 and is shared with the neighboring Church of St. Clement. Therefore, the puzzle does not actually depict the Church of St. Clement. The Church of St. Clement is one of the most significant examples of High Baroque architecture in Prague and serves as the main cathedral of the Apostolic Exarchate of the Czech Republic. It was built by the Jesuits between 1711 and 1715 on the site of a smaller Romanesque-Gothic church, next to which a Dominican monastery was founded in the 13th century.
The tall church tower that rises in the middle belongs to the Church of St. Salvator, which is part of the Clementinum complex.
