This memorial consists of a circular plaza with the American flag in the center of the plaza. A small garden surrounds the flag. Five benches on the outer circle. Behind the flag is a monument which is surrounded by five flags of the Armed Services. The plaza is composed of engraved pavers bearing the names of veterans and branches in which they served.
There is a beautiful water feature behind the Smithtown Veterans Plaza sign.

The Monument reads: WE DEDICATE THIS MONUMENT TO THE MEN AND WOMEN VETERANS OF SMITHTOWN PAST, PRESENT AND FUTURE WHO DISTINGUISH THEMSELVES BY THEIR HONORABLE SERVICE IN OUR COUNTRY'S MILITARY LET US NEVER TAKE FOR GRANTED OUR CHERISHED DEMOCRACY AND FREEDOMS. LET THIS MONUMENT SERVE AS A PERPETUAL REMINDER TO ALL WHO VISIT HERE OF THEIR INVALUABLE SERVICE AND SACRIFICE. On the bottom of the monument is engraved the Seals of all Military Branches. On the back side of the Monument reads: A VETERAN'S PRAYER: DO NOT STAND AT MY GRAVE AND WEEP, I AM NOT THERE. I DO NOT SLEEP, DO NOT STAND AT MY GRAVE AND CRY I AM NOT THERE. I DID NOT DIE AS LONG AS I AM IN THE MEMORY OF THOSE THAT LOVE ME STILL, FOR MY LEGACY TO YOU IS TO ENJOY FREEDOM TRUE THAT IS MY ONLY WILL.

This Monument is in a small parcel of land on Main Street between a Bank and CVS. Parking can be found at the bank and CVS and a brick pathway to the memorial.
