The entrance to the cemetery has a wrought iron arch or gateway that is attached to two metal posts and sunk in concrete on two brick and concrete bases. The arch reads Clay Sink Cemetery with the year 1873 in the center. A citizen memorial is on one of the brick bases honoring Harrison and Martha Ann Slaughter.
