This bird house is located behind a historic marker and on the side of the walkway to the house.
It is a hexagon style, resembling a Gothic Revival structure, with steep roof and faux dome on top.

House was unoccupied when I visited, the bird house anyway. The Human structure was occupied. This is an old bird house, the wood is weathered, and needs, desperately, a coat of paint.
