This Blue Start Memorial plaque is located in Chestnut Ridge Park near Ellicott, New York, just south of Buffalo. The plaque is located in Flagpole Garden next to the Chestnut Ridge Casino building.

The plaque was placed in a ceremony on July 29th, 2015. It was the first Blue Star plaque in Erie County.
