K letecké katastrofe došlo u 8.slp dne 21.5.1964 v prubehu letecko-taktického cvicení.
Dvojice vlétla v prostoru Frenštát pod Radhoštem do nízké oblacnosti. V kopcovitém terénu hrozilo nebezpecí srážky s terénem a velitel dvojice velel stoupat. Nad souvislou vrstvu mraku však vylétl sám. Mjr.Vlastimil Zaydlar se již neobjevil a neozval. Narazil ve výšce 800 metru do severozápadního úbocí Lysé hory a v kabine letounu Mig-19S trupového oznacení 0419 zahynul.

The air disaster occurred at the 8th SLP on May 21, 1964 during an air-tactical exercise.
The pair flew into the low clouds in the area of Frenštát pod Radhoštem. In the hilly terrain there was a danger of a collision with the terrain and the commander of the pair ordered to ascend. However, he flew over a continuous layer of clouds alone. Maj. Vlastimil Zaydlar did not appear and respond. He crashed at an altitude of 800 meters into the northwestern slope of Lysá hora and died in the cabin of the Mig-19S fuselage 0419.
