Located in Port Charlotte, Florida is the Douglass T. Jacobson State Veterans Home. On the grounds of the home are several memorials dedicated to service members and civilians. A dedicated tree is located on the lawn of the entrance to the veterans home.
The bronze plaque is mounted in a large sandstone and reads as follows:
