The Omak Avenue bridge (AKA Highway 155 bridge) is the southwesterly of two bridges in Omak, the other one being on Highway 97. F 21 is in the top of the south guardrail on the west end of the bridge, 1.3 metres (4.3 feet) east of the west end.

IN OMAK, AT STATE HIGHWAY 155 BRIDGE OVER THE OKANOGAN RIVER, IN TOP OF THE SOUTH CONCRETE GUARD RAIL 1.3 METERS (4.3 FT) EAST OF THE WEST END, 1.35 KM (0.85 MI) NORTHWEST OF THE CROSSING OF THE BURLINGTON NORTHERN RAILROAD CROSSING, 1.7 METERS (5.5 FT) SOUTH OF THE SOUTH BRIDGE CURB.
THE MARK IS 1 M ABOVE THE HIGHWAY.
