The tree stump headstone of Annie Lee Ford stands on four horizontal logs engraved with a calla lily. At the top, the trunk divides into two trunks. The Woodmen Circle seal is unusual as it is not a circle, but the bottom half of a shield. The inscription is engraved on a smooth panel and reads:
