The mark is a a brass disk set close to the ground, southwest of the granite post that marks the corners of Connecticut, Massachusetts, and Rhode Island.

In Thompson, just southwest of a granite post marking the meeting of the states of Connecticut, Massachusetts, and Rhode Island, is a brass survey marker designated "TRI-STATE."
The marker is located within state-owned woods. The datasheet has the following text about the mark:

To get to this mark, from Thompson, go east on E Thompson Road to a place where there is a turnout where there is a sign for the Airline Trail State Park and a kiosk. Park there, cross the street going east and enter the trail, going toward Massachusetts. Continue walking on this graded trail to a place where there is a post marking a path on the right to the Tri-State Marker, .3 miles. Turn right (south) on this trail, passing a granite post marking the boundary of Massachusetts and Connecticut, and follow this marker to the Tri-State granite post. From there, walk approximately 25 feet roughly southwest to the brass marker located about six inches above ground level.
I did not recover the reference disks. One may have been by the granite post - it is presently a drill hole north east of the survey mark and north of the granite post.
The brass disk is in fair condition. There are many pits on the surface. A corner of the concrete block that it is set in has been broken off.
