The bronze plaque is on an angled, almost rectangular, roughly hewn block of granite, at the start of a curving concrete path to the South West Rocks Cenotaph, just south of Horseshoe Bay. The plaque reads:

In memory of those who gave their lives

5th August 1914 to 11th November 1918
Their sacrifice and contribution to the Nation will never be forgotten.
